• 1. Download Wipes, Paint or Soil COC, Air COC or Drinking Water COC as applicable.
• 2. Print, complete, and send with your sample.

• PLEASE NOTE: For all submittals to Accurate Analytical Testing LLC., a Chain of Custody form (COC) MUST be completed, signed, dated and returned with all sample submitted to for laboratory analysis. All COC forms must be completely filled in to avoid potential delays in processing your work.
